Update 4/17/2026 regarding new application forms. In light of concerns on behalf of some of our larger providers, WV CARES will accept the old Self-Disclosure and Consent forms through 6/30/2026. Beginning 7/1/2026, the old forms will be refused by our Fitness Determination Specialist and the determination will be marked Missing Self-Disclosure.


All forms under Applications -> Application Forms have been updated with the state of WV Office of Inspector General seal/logo. All providers are expected to now use the updated forms. This includes the Self-Disclosure & Consent form that must be uploaded to each application. Please destroy any old versions of our forms that you may have on hand.



UPDATE as of 4/17/2026: We believe all applications from 3/26/2026 - 3/31/2026 that could be connected, have been. If you believe that you still have applications being affected by the system issues DURING THE REFERENCED TIME FRAME, please reach out as previously instructed. We are now working on any fee discrepancies that have been identified. We again ask for your continued patience.

ATTENTION ALL PROVIDERS: A system update on 3/27/2026 was missing a component and caused our system to fail in working the way it is intended. This failure affected over 1000 applications that were entered from 3/27/2026 until 3/31/2026. We were informed by our developers that they fixed the issue going forward but all applications from 3/27/2026 until 3/31/2026 will have to be manually fixed by the WV CARES staff. We will be reaching out by email to those affected by this issue. In the meantime, if you entered an application during the referenced time frame and it is still outstanding, please send an email to wvcares@wv.gov. In the subject line: Attention Deputy Director - Connection Issues. In the body of the email please give the name, DOB, and application number to be investigated. We appreciate your patience, as we are working as quickly as possible to fix this situation, pending further assistance from our developers.



Beginning 03/26/2026 – ALL providers will be required to upload their registry research results for the WV Sex Offender Registry and the National Sex Offender Public Website, proving that these tasks were completed.


IF YOUR APPLICANT APPEARS ON ANY OF THE REQUIRED REGISTRIES, THEY ARE NOT ELIGIBLE FOR EMPLOYMENT, REGARDLESS OF THEIR JOB TITLE OR POSITION FOR WHICH THEY ARE APPLYING.

Any outdated form dated April 1, 2026 or after, will require resubmission on the form which contains the WV OIG Seal/Logo. These forms are available under Applications tab, Application Forms in the WV CARES system. Please destroy all previous versions of WVC variance forms you have on hand.


PLEASE READ IMPORTANT MESSAGE:

IF AN INDIVIDUAL SHOWS UP ON ANY REGISTRY, YOU MUST VERIFY THE INFORMATION AND IF IT IS A MATCH YOU MUST WITHDRAW THE APPLICATION IMMEDIATELY AND INFORM THE INDIVIDUAL THAT THEY ARE NOT ELIGIBLE TO WORK UNDER THE WV CARES CODE AND ARE NOT ELIGIBLE FOR A VARIANCE/APPEAL. ANY PROVIDER THAT KNOWINGLY CLEARS AN INDIVIDUAL THAT SHOWS UP ON ANY REGISTRY, ONCE IT IS VERIFIED AS THE SAME INDIVIDUAL, IS IN VIOLATION OF THE WV CARES RULE AND WILL BE SUBJECT TO PENALTIES.


Attention: Please be advised that our system will close all applications on your At A Glance that are in the Not Yet Submitted, Eligibility Determination in Process, Determination Available and Action Needed, and Eligibility Determination Complete that are more than 180 days old with no actions taken on your part.



7.31.2025 – ATTENTION: ALL PROVIDERS THAT HAVE DESIGNATED A BILLING ACCOUNT FOR YOUR PAYMENT OF THE WV CARES ADMINISTRATIVE/APPLICATION FEE.


BEGINNING 8.15.2025 the “CREATE PROVIDER BILLING STATEMENT” will no longer be available as an option. You are to print your “WVDHHR PROVIDER BILLING REPORT” and mail this report in with your check or money order to the payment address listed on the WVDHHR PROVIDER BILLING REPORT. Please update anyone in your accounts payable department to take note of the only address where payments will be received by WV CARES. All other addresses are obsolete. Following this new process is imperative for the WV CARES Accountant to properly credit your account in a timely manner.

WV CARES operates under the WV Code §16B-15-1, et seq.

The WV CARES legislative rule can now be viewed under §71-11-1, et seq. These documents will be available under HELP in the top right of the website.


Please be advised that there is now a service fee for all payments processed through our 3rd party credit/debit card processor. The amount of the fee and the total you will be charged is on the screen prior to you entering any of your information. WV CARES does not receive any of the service fee, nor did we have any control in the implementation of it.



IMPORTANT MESSAGE:

The NNAR (National Nurse Aide Registry) was decommissioned in the WV CARES system after business hours on Friday, September 13, 2024.


This change affects our state's application workflow. Changes include:

  • State Registry Requirement: Your state's Nurse Aide Registry (NAR) will now be a mandatory part of the process.
  • State-Specific Searches: The registries of other states will only be accessed if they are relevant to the current or prior addresses provided in the application. This is consistent with how out-of-state registries are currently handled.
  • Public Web Searches: All nurse aide registry checks (except those against your own state registry) will now be conducted via public web searches, rather than through automated matches.

This also means that the NNAR will no longer appear on your required monthly Registry Recheck Reports.


05.03.2024 The application/administrative fee has increased to $25.00 per WV CARES legislative rule 64 CSR 123 which is in effect as of TODAY.


7/24/2023 - We have been informed by the WVSP that IdentoGo WILL ACCEPT the temporary paper identification card/driver's license issued from the DMV. If you have a specific location that will not accept these documents, please provide us with the fingerprinting location and the agent's identifying information. If possible, have your applicant contact WVC while at the fingerprinting site, so we can attempt to resolve the confusion immediately, and their fingerprinting can occur at that time. Idemia/IdentoGo will no longer accept expired forms of Identification.
